Job Description

Overview

The Referral Tracking Coordinator at Comprehensive Community Health Centers Inc. is responsible for managing the tracking and documentation of patient referrals and consult reports. This role involves regular communication with specialty offices to ensure appointments are scheduled and follow-ups are conducted to maintain referral integrity.

Key responsibilities

  • Obtain, review and request authorizations and referrals as needed.
  • Monitor and track patient referrals and document efforts in i2i systems and EMR.
  • Contact patients to ensure specialist appointments are scheduled.
  • Follow up with patients and specialists to confirm appointments.
  • Obtain consult reports to close the referral loop and document all communications.
  • Communicate issues and amendments to the requesting provider.
  • Serve as liaison between patients, clinic staff, providers, and outside agencies.
  • Perform other duties as assigned.

Required skills

  • High School Diploma or GED
  • 3-5 years of experience in a medical office setting
  • Bilingual in Armenian and English
  • Strong documentation skills
  • Ability to multi-task and prioritize
  • Experience in a team-oriented environment
  • Excellent interpersonal communication skills
  • Proficient knowledge of Microsoft Outlook
  • Ability to learn new software programs
