Overview

The Patient Growth Specialist (Outreach Executive I) at Oak Street Health is responsible for connecting Medicare-eligible seniors with the organization in Phoenix, Arizona. This community-facing role focuses on generating leads, building relationships, and fostering patient growth by acting as a brand ambassador for the unique care model offered by Oak Street Health.

Key responsibilities

  • Generate high-quality leads through community events and localized outreach.
  • Identify and convert leads via cold calling, digital channels, and referrals.
  • Engage prospective patients through email, phone, and face-to-face networking.
  • Document all customer interactions and sales activities within the CRM.
  • Serve as the liaison between clinical teams and new patients.
  • Address and resolve prospect concerns with empathy.
  • Maintain consistent touchpoints with prospects for community presence.

Required skills

  • 2+ years in Sales or Healthcare environments
  • Fluency in English and Spanish
  • Cold calling and digital lead management experience
  • Exceptional verbal and written communication skills
  • Skilled at managing multiple priorities in a fast-paced environment
  • Valid driver's license and reliable transportation
  • Basic knowledge of Microsoft and/or Google Suite
  • Experience with CRM and/or EMR software
