Overview

This member of the multidisciplinary team has training in one of the mental health professions (social work, rehabilitation counseling, or psychology) and clinical skills and experience in assessing, planning, developing, coordinating, and providing treatment, rehabilitation, and support services to program participants and families with severe and persistent mental illnesses under the clinical supervision of the Program Manager. This member functions as a clinical member of the multidisciplinary team and provides treatment, rehabilitation, and support services to the OnTrack program, a self-contained integrated multidisciplinary team providing compressive, intensive, community-based treatment to adolescents and young adults who are within two years of experiencing the onset of non-affective psychosis. Essential Duties/Requirements/Activities: Ability to abide by principles of EEO compliance and a workplace of dignity and respect.Ability to work cooperatively in a group/team setting.Ability to show respect to others.Ability to take guidance and direction from supervisors.Ability to report to work with clean hygiene.Ability to adhere to company /program dress code standards.Ability to arrive/report to work on time and ready to work.Ability to communicate with others.Attends all scheduled work hours, meeting, training, and other center functions, including on-call as required.Provide individual, family and group therapy for an assigned group of people including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans; provide the individual with the support necessary for symptom management; ensure immediate changes are made in treatment plans as persons' needs change; educate and support persons' families; and advocate for persons' rights and preferences.Complete all daily progress notes in Credible and assessments in a timely manner as mandated in the program policies and procedures.Participate in the completion of a comprehensive assessment of all program participants admitted to the program.Consult with community agencies and families to maintain coordination in the treatment process.Provide on-call crisis intervention as assignedDocument persons' progress to maintain a permanent record of person's activity according to established methods and proceduresParticipate in weekly staff organizational meetings and treatment planning review meetings. Conduct needs assessments. Engages in safety planning and wellness management planning with participants. Conducts clinical assessment to determine the participant's level of social functioning and substance use via a shared decision making (SDM) framework. Provides Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psychoeducation. Provides psychoeducation regarding first episode psychosis (FEP) and the goals of the program. Provides supportive therapy. Qualifications: Education: Master's degree in one of the core Behavioral Health Sciences or in an equivalent Master's program. Experience: Prefer experience in a clinical or rehabilitation setting with persons with severe and persistent mental illnesses, and experience providing therapy to children and adolescents.
