Overview

Leisure Care is seeking a Health and Wellness Nurse to enhance the quality of life for older adults in their Melbourne community. The nurse will be responsible for health assessments, medication management, and care coordination for residents, while also supporting staff through education and supervision.

Key responsibilities

  • Conduct health assessments and create service plans for community residents
  • Manage medication and coordinate care for resident health needs
  • Assist the Health & Wellness Director with staff supervision and education
  • Provide health-related expertise to residents, families, staff, and referral sources

Required skills

  • Two years of assisted living experience preferred
  • Current RN or LPN/LVN license in good standing
  • Knowledge of geriatric care, dementia care, and state regulations
