Overview

Texas Children's Hospital is seeking a Case Manager to work in a fast-paced environment, focusing on patients with chronic conditions such as sickle cell disease, diabetes, and asthma. The role involves assessing, planning, and coordinating healthcare services to meet the needs of individuals and their families.

Key responsibilities

  • Proactively identify members in need of case management through various referrals and consultations.
  • Assess members' biophysical, psychosocial, and environmental needs.
  • Communicate continuously with members, families, and medical staff to facilitate care.
  • Utilize motivational interviewing techniques with members and families.
  • Conduct home visits and clinic or hospital visits as needed.
  • Liaise with community organizations to support maternity and pediatric populations.
  • Educate providers on compliance and policies related to managed care and Medicaid.
  • Perform provider visits to discuss member program adherence.
  • Monitor outcome data to identify improvement opportunities.

Required skills

  • Diploma in nursing or associate's degree in nursing or a related field
  • Bachelor's degree in nursing preferred
  • Registered Nurse license by Texas Board of Nursing or Nursing Licensure Compact
  • Case management certification preferred
  • 3 years of clinical experience in pediatrics, home care, or case management
