Overview

Molina Healthcare is seeking a Care Manager (BH Licensed) to support care management and coordination activities. This role involves collaborating with a multidisciplinary team to deliver integrated member care across various services, particularly focusing on behavioral health.

Key responsibilities

  • Complete comprehensive behavioral health assessments of members
  • Develop and implement care plans in collaboration with members and healthcare professionals
  • Conduct telephonic, face-to-face, or home visits as needed
  • Monitor care plans for effectiveness and suggest changes
  • Maintain an ongoing member caseload for regular outreach
  • Promote integration of services including behavioral health and community resources
  • Facilitate interdisciplinary care team meetings
  • Use motivational interviewing to support and motivate members
  • Assess barriers to care and provide coordination assistance

Required skills

  • At least 2 years of healthcare experience, preferably in behavioral health
  • Licensed behavioral health clinician (LCSW, LMHC, LMFT, LMSW, etc.)
  • Valid and unrestricted driver's license and reliable transportation
  • Experience with severe mental health concerns and substance use disorder
  • Knowledge of whole person care principles
  • Data entry skills and experience utilizing a clinical platform
  • Excellent verbal and written communication skills
  • Proficiency in Microsoft Office suite
