Job Description

Overview

Supplemental Health Care is seeking Home Health Physical Therapists (PTs) to provide compassionate, patient-centered care in Zanesville, Ohio. This role supports patients in their homes and involves conducting evaluations, developing therapy plans, and collaborating with other healthcare professionals.

Key responsibilities

  • Perform initial and ongoing PT assessments and develop individualized plans of care
  • Provide skilled interventions for strengthening, endurance, pain management, and fall prevention
  • Conduct gait and balance evaluations, home safety assessments, and mobility training
  • Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies
  • Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care
  • Document all care in compliance with Medicare, OASIS-E, and agency standards

Required skills

  • Active, unrestricted Physical Therapist (PT) license in the state of assignment
  • Current BLS certification (AHA)
  • 1+ year of recent home health experience
  • Experience conducting PT evaluations and developing therapy plans
  • Familiar with Medicare-certified home health guidelines and OASIS-E documentation
  • Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
  • Reliable transportation with valid auto insurance and ability to travel between patient homes
  • Clear background check and up-to-date health clearance
