Job Description

Overview

Supplemental Health Care is seeking Home Health Physical Therapists (PTs) to deliver compassionate, patient-centered care in Upper Marlboro, Maryland, and nearby areas. The role requires providing evaluations, developing therapy plans, and ensuring compliance with Medicare guidelines while maintaining a focus on patient education and mobility training.

Key responsibilities

  • Perform initial and ongoing PT assessments and develop individualized plans of care
  • Provide skilled interventions for strengthening, endurance, pain management, and fall prevention
  • Conduct gait and balance evaluations, home safety assessments, and mobility training
  • Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies
  • Collaborate with RNs, OTs, SLPs, and physicians for coordinated, interdisciplinary care
  • Document all care in compliance with Medicare, OASIS-E, and agency standards

Required skills

  • Active, unrestricted Physical Therapist (PT) license in Maryland
  • Current BLS certification (AHA)
  • 1+ year of recent home health experience
  • Experience conducting PT evaluations and developing therapy plans
  • Familiarity with Medicare-certified home health guidelines and OASIS-E documentation
  • EMR proficiency with Homecare Homebase, NetSmart, or Epic
  • Reliable transportation with valid auto insurance
