Overview

Supplemental Health Care is seeking Home Health Physical Therapists (PTs) in Brooksville, Florida, to deliver compassionate, patient-centered care in home health settings. The company, with over 40 years of healthcare staffing experience, aims to connect clinicians with top home health partners while ensuring adherence to Medicare guidelines.

Key responsibilities

  • Perform initial and ongoing PT assessments and develop individualized plans of care
  • Provide skilled interventions for strengthening, endurance, pain management, and fall prevention
  • Conduct gait and balance evaluations, home safety assessments, and mobility training
  • Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies
  • Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care
  • Document all care in compliance with Medicare, OASIS-E, and agency standards

Required skills

  • Active, unrestricted Physical Therapist (PT) license in the state of assignment
  • Current BLS certification (AHA)
  • 1+ year of recent home health experience
  • Experience conducting PT evaluations and developing therapy plans
  • Familiarity with Medicare-certified home health guidelines and OASIS-E documentation
  • Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
  • Reliable transportation with valid auto insurance
  • Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
