With a focus on holistic care, you'll collaborate with healthcare providers and families to ensure the highest quality of support for patients.

Key responsibilities

  • Educate patients and families on health management and care plans
  • Conduct comprehensive patient assessments and evaluations
  • Implement and manage patient care plans tailored to individual needs
  • Collaborate with interdisciplinary teams to promote continuity of care
  • Organize and prioritize patient care tasks based on needs
  • Engage in professional development and achieve personal growth goals
  • Conduct patient assessments to determine care priorities
  • Utilize healthcare organization standards in care delivery

Required skills

  • 2-3 years of Med/Surg experience (preferred)
  • Strong wound care and infusion skills
  • ICU or ED experience (considered)
  • OASIS experience (preferred but not required)
  • Excellent time management and independent work abilities
  • Current nursing license in North Carolina
  • Ability to travel to patient homes
  • Experience with medical-surgical protocols
