Overview

Interim HealthCare is seeking an RN Case Manager in Greenwood, Indiana, offering a sign-on bonus of $7,500. This role is designed for nurses who enjoy bedside care and want to make a meaningful impact in patients' lives while also benefiting from flexible scheduling and work-life balance.

Key responsibilities

  • Conduct in-person patient interviews and comprehensive physical assessments
  • Oversee the implementation and ongoing assessment of the patient's plan of care
  • Manage home health aides, LPNs, RNs, and other caregivers
  • Communicate patient conditions and collaborate with providers for evolving care needs
  • Educate patients and families on home health care procedures such as wound care and medication management
  • Promote preventative care to decrease readmissions and ensure continuity of care

Required skills

  • Current unrestricted license to practice as a Registered Nurse (RN)
  • Current CPR/AED/BLS/First Aid certification
  • Reliable transportation to care sites
  • One year of experience as a Registered Nurse in home health or similar setting
  • Preferred experience in case management/utilization review
  • Preferred OASIS experience
  • Preferred practical trach and/or ventilator experience
