Overview

The RN Case Manager at Interim HealthCare provides vital support for patient services in Westmoreland County and East Pittsburgh. This role involves conducting assessments, managing care plans, and ensuring continuity of care for patients in a home health setting.

Key responsibilities

  • Conduct in-person patient interviews and comprehensive physical assessments
  • Oversee the implementation and ongoing assessment of the patient's plan of care
  • Manage home health aides, LPNs, RNs, and other caregivers
  • Communicate patient conditions and collaborate with healthcare providers
  • Educate patients and families on home health care procedures
  • Promote preventative care to decrease readmissions

Required skills

  • Current unrestricted RN license
  • CPR/AED/BLS/First Aid certification
  • Reliable transportation
  • One year of professional RN experience in home health or similar setting
  • Previous case management/utilization review experience preferred
  • OASIS experience preferred
