Overview

CarePartners is seeking a Home Health Physical Therapist Case Manager to join their Franklin, NC team. This role involves providing and directing physical therapy services, collaborating with care management and interdisciplinary teams to ensure high-quality care for patients, and maximizing positive financial outcomes through effective case management practices.

Key responsibilities

  • Perform assessments and treatment planning for patients under physicians' orders
  • Collaborate with care management and interdisciplinary teams
  • Monitor patient response to treatment and modify plans as necessary
  • Develop and implement home programs to enhance patient performance
  • Supervise and direct Physical Therapy Assistants in treatment plan execution
  • Participate in patient, family, and staff education
  • Submit billing and documentation in a timely manner
  • Adhere to infection control and confidentiality standards

Required skills

  • Bachelor of Science in Physical Therapy
  • National Certification Examination for Registered Physical Therapist
  • State license to practice physical therapy
  • Valid NC Drivers License
  • Ability to work varying hours, including weekends and holidays
  • Bilingual in Spanish preferred
