Overview

Saint Alphonsus Health System is seeking a Patient Navigator (LPN) in Boise, Idaho. This full-time role involves coordinating care for patients within the organization's health plan and value-based care programs, ensuring they receive high-quality, clinically appropriate services while collaborating with various healthcare stakeholders.

Key responsibilities

  • Conduct outreach to patients following Emergency Department visits for continuity of care.
  • Perform clinical chart reviews to identify necessary care directions and recommendations.
  • Coordinate and monitor specialist referrals and community resource connections.
  • Collaborate with providers, health plans, and care teams to align care delivery with performance goals.
  • Serve as a resource for questions about level of care, network utilization, and member benefits.
  • Validate member eligibility and facilitate authorization processes.
  • Identify trends in access or scheduling and recommend improvements.
  • Participate in quality improvement initiatives.
  • Maintain compliance with HIPAA and organizational policies.

Required skills

  • Licensed Nurse in Idaho (LPN)
  • 3+ years experience in health plans or provider networks
  • 5 years clinical experience preferred
  • Knowledge of care navigation and coordination
  • Experience with clinical documentation and referral management
  • Strong collaboration skills with care teams and providers
  • Understanding of healthcare compliance and regulations
