Guided by our H.E.A.R.T. values, we are committed to transforming lives and communities.About the Role:As an Outreach Coordinator, you will serve as a bridge between the community and essential behavioral health services. This role is highly community-focused and allows you to meet individuals where they are-whether in shelters, on the street, or in other community settings-and help guide them toward care and resources.You'll work both independently and alongside a multidisciplinary team to assess needs, remove barriers, and connect individuals to housing, treatment, and supportive services.
