Overview

One Stop Recruiting is seeking an experienced Nurse Practitioner (NP) for their outpatient primary care practice in Stockton, CA. The position focuses on delivering comprehensive, patient-centered care while fostering long-term relationships and managing chronic conditions in a collaborative environment.

Key responsibilities

  • Deliver comprehensive primary care including well visits, sick visits, and annual physicals
  • Manage chronic conditions with a focus on long-term disease and medication management
  • Provide ongoing preventive care and patient education
  • Serve as an autonomous provider for an assigned patient panel
  • Coordinate care with an interdisciplinary team to improve patient outcomes
  • Maintain accurate and timely clinical documentation
  • Support continuity of care through follow-up and care coordination
  • Address complex patient needs in an outpatient setting

Required skills

  • Minimum of 2 years of primary care experience as a Nurse Practitioner
  • Credentialed or eligible for Medicare and Medicaid participation
  • Comfortable functioning as a sole provider
  • Strong clinical judgment and patient management skills
  • Compassionate approach to underserved populations
