Overview

Kaiser Permanente is seeking a Medical Social Worker/LICSW to provide a range of clinical social work services in various settings including primary care, inpatient care, and home health. The role involves evaluating psychosocial assets and coordinating care while providing support to patients and families to maintain their independence.

Key responsibilities

  • Provide social work interventions to patients and their families
  • Conduct psychosocial assessments and brief individual or family counseling
  • Advocate for patients and link them to resources
  • Coordinate care with a multidisciplinary team and medical staff
  • Assess patient and family needs for care and communicate effectively
  • Collect and evaluate relevant data throughout the duration of care
  • Modify care plans based on ongoing assessments

Required skills

  • Masters degree in Behavioral Science or Social Work
  • Licensed Marriage and Family Therapist (Washington)
  • Licensed Mental Health Counselor (Washington)
  • Licensed Advanced Social Worker (Washington)
  • Licensed Independent Clinical Social Worker (Washington)
  • Excellent written and verbal communication skills
  • Experience in a multidisciplinary team
  • Knowledge of social case management and conflict resolution
  • Familiarity with Microsoft Office and electronic charting
