Key ResponsibilitiesConduct comprehensive physical assessments, identifying patient needs and developing personalized treatment plans that reflect each individual's goals.Implement therapeutic interventions involving exercises, manual therapy techniques, and modalities aimed at improving patients' physical health and mobility.Monitor patient progress through ongoing evaluations and adjust treatment plans accordingly to maximize therapeutic outcomes.Educate patients and their families about treatment protocols, expected outcomes, and strategies for maintaining physical health outside of therapy sessions.Collaborate with other healthcare providers, such as medical doctors, psychologists, and social workers, to create holistic care plans that integrate physical and mental health treatments.Maintain accurate and comprehensive patient records, ensuring compliance with ethical, legal, and organizational standards. Patient Population / Client Focus The patients you will serve at Top Acres include individuals of all ages who may be experiencing physical limitations, recovering from injuries, or managing chronic conditions. As a Physical Health Therapist, you will address a wide range of conditions related to mobility, pain management, and overall wellness, contributing significantly to each patient's journey towards improved health. QualificationsA Bachelor's Degree in Physical Therapy or a related field.Current and valid state licensure as a Physical Therapist.Proven experience in physical therapy practices, particularly in an outpatient setting.Exceptional communication and interpersonal skills conducive to building strong therapeutic relationships with patients and collaborating with healthcare teams.Knowledge of evidence-based practice and familiarity with contemporary therapeutic modalities.
