Key ResponsibilitiesConduct thorough assessments of patients' physical health and functional capabilities, utilizing appropriate assessment tools and methodologies to develop individualized treatment plans.Implement therapeutic interventions that address and improve patients' physical health conditions, maximizing their functional independence and quality of life.Monitor and document patient progress, adjusting treatment plans as required to ensure optimum recovery outcomes.Educate patients and their families about physical health issues, the rehabilitation process, and preventative strategies to enhance overall well-being.Participate in interdisciplinary team meetings to collaborate with medical, psychological, and support staff, ensuring integrated care approaches that align physical health with mental health support.Engage in patient education workshops and community outreach initiatives to raise awareness on the importance of physical health and rehabilitation.
