Job DescriptionJob DescriptionPosition Description: The Law Enforcement Liaison will be working with the Designated Collaborating Organization, however is still employed by Elevate Housing Foundation. This position is working with local law enforcement agencies as a crisis responder and will be assisting with mental health crisis calls along side law enforcement. Job Responsibilites Assist clients in accessing supports, services, and resources for their behavioral health recoveryRespond to 911 calls for assistance with support of Law EnforcementRespond to calls for Mobile Crisis Services when availableComplete documentation in regards to any mobile crisis callsProvide information and referrals for individuals seeking assistanceProvide crisis services, de-escalation, and follow-up for individuals per regulated timelinesFacilitate comprehensive habilitation enrollment and disenrollment processesDevelop and implement crisis plans/safety plansPerform ongoing monitoring of the care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordinglyPromote integration of services for patients including behavioral health care and long-term services and supports, home and community to enhance the continuity of care for individualsAssess for medical necessity and authorize all appropriate servicesConduct face-to-face or home follow-up visit when neededUse motivational interviewing and clinical guideposts to educate, support and motivate change during individual contactsAssess for barriers to care, provide care coordination and assistance to patients to address psycho/social, financial, and medical obstacles and concernsIdentify critical incidents and develop prevention plans to assure patient's health and welfareBenefits Competitive salary Medical, Dental and Vision plans Vacation plan Paid Time Off QUALIFICATIONS EDUCATION/CERTIFICATION:Bachelor's degree in health and human services or psychology from an accredited college preferredCertification in Mental Health First Aid, Critical Incident Training (CIT), First Aid, CPR, Crisis Prevention Institute (CPI) is valued.EXPERIENCE REQUIRED:2 years' experience working with individuals with behavioral health needs.Forensic setting preferred.SKILLS/ABILITIES:Office 365, Good Driving Record and Valid Iowa Driver's LicensePHYSICAL ACTIVITIES AND REQUIREMENTS OF THIS POSITION:TALKING: Good verbal and communication skills.AVERAGE HEARING: Average hearing with or without adaptive devices.REPETITIVE MOTION: Data entry.FINGER DEXTERITY: Average dexterity for data entry and management.AVERAGE VISION: Average vision with or without adaptive devices.PHYSICAL STRENGTH: Minimal strength requirements. Comfortably lift and carry 10-15 pounds.WORKING CONDITIONS/HOURS: Most work will be performed in community settings. Will ride patrol with law enforcement on regular basis. Hours may vary day to day including working 1st shift, 2nd shift or 3rd shift hours as required and needed with law enforcement.MENTAL ACTIVITIES AND REQUIREMENTS OF THIS POSITION:REASONING ABILITY: Strong reasoning ability required to assess and deescalate crisis situations.MATHEMATICS ABILITY: Average mathematic ability required.LANGUAGE ABILITY: Strong language skills required for effective written and verbal communication.
