Overview

The Hospice and Palliative Nurse Practitioner at Vitas Healthcare plays a crucial role in delivering palliative care and symptom management for patients with serious illnesses. Working as part of an interdisciplinary team, the NP will consult on care plans, educate staff and the community, and ensure that patient and family goals are clarified and met.

Key responsibilities

  • Facilitate clarification of patient and family goals of care
  • Assist with diagnosis, treatment, and management of health conditions
  • Educate facility staff and community about palliative care
  • Attend and participate in internal team meetings
  • Collaborate with referring physicians and healthcare teams
  • Prepare and maintain accurate patient records and documentation
  • Prescribe medications and order diagnostics/treatments as needed
  • Interpret and communicate diagnostic findings
  • Collaborate and supervise palliative care team members
  • Assist in developing clinical practice guidelines and standards
  • Utilize a customer service approach in all interactions
  • Participate in on-call rotation as needed

Required skills

  • Minimum 2-5 years nursing experience
  • Experience in oncology, hospice, geriatrics, intensive care
  • Knowledge of interdisciplinary team concepts
  • Experience in patient education and care planning
  • Strong communication skills, both verbal and written
  • Working knowledge of computers and software like Outlook, Excel, and Word
  • Ability to manage pain and other symptoms for advanced illnesses
