Overview

The Clinical Care Manager (RN) at VitalCaring Group in Madisonville, Texas, plays a crucial role in leading care coordination and guiding clinical decisions to ensure exceptional patient outcomes in home health care. This position emphasizes ownership and collaboration in a fast-paced environment, aimed at delivering meaningful impacts on patients' lives.

Key responsibilities

  • Lead and coordinate care across the full patient journey
  • Manage physician orders, documentation, and workflows within the EMR
  • Conduct ongoing patient assessments and ensure plan-of-care alignment
  • Review clinical reports and take appropriate action
  • Lead case conferences, recertifications, and discharge planning
  • Ensure compliance with clinical, state, and federal regulations
  • Serve as a key liaison between physicians, patients, families, and internal teams
  • Support care delivery through in-home visits as needed

Required skills

  • Active RN license in state of practice
  • Graduate of an accredited nursing program (BSN preferred)
  • Home Health experience strongly preferred
  • Experience in care coordination, case management, or clinical leadership preferred
  • Strong clinical judgment and ability to manage multiple priorities
  • Confident communicator with ability to collaborate across disciplines
  • Comfortable working independently in a fast-paced environment
  • Strong organizational skills with attention to detail in documentation and compliance
