Job Description

Overview

The Assertive Community Treatment Team Services Coordinator at Pathways Vermont is responsible for providing community-based mental health services to individuals facing mental health challenges, histories of homelessness, and substance use issues. This role involves working as part of an interdisciplinary team to promote stability and recovery for clients through advocacy, support, and resource navigation.

Key responsibilities

  • Engage in relationship building and rapport with clients using client-centered and harm reduction approaches
  • Collaborate with clients to maintain a comprehensive, Medicaid-compliant service plan
  • Assist clients in navigating the housing path and developing life skills
  • Maintain knowledge of community resources and assist clients with referrals
  • Provide one-on-one support regarding life skills, housing concerns, and health maintenance
  • Coordinate and monitor referrals to community services and advocate for client participation
  • Educate clients about benefits and entitlements
  • Participate in 24/7 on-call services rotation
  • Maintain written and computerized records and compile reports

Required skills

  • Experience in mental health and/or harm reduction agency
  • Knowledge of issues facing individuals with mental health challenges or substance use
  • Understanding of the criminal justice system's impact on individuals with mental illness
  • Excellent communication, writing, and interpersonal skills
  • Ability to work independently and prioritize tasks
  • Strong problem-solving skills
  • Knowledge of community resources for mental health and substance use
  • Valid driver's license and reliable personal vehicle
  • Ability to lift and carry 50 pounds
  • Experience or education in nursing, substance abuse support, or benefits coordination is ideal
