Job Description

Overview

The Youth Mental Health Case Manager at PermiaCare is responsible for providing evidence-based services to children and families dealing with mental health and behavioral issues. The role involves assisting youth in accessing essential services and developing skills to enhance their community functioning and recovery goals, with collaboration across various agencies.

Key responsibilities

  • Complete and update Recovery Plans and Uniform Assessment Forms.
  • Develop and implement Crisis/Safety Plans.
  • Provide face-to-face skills training for youth and parents.
  • Conduct routine case management and monitoring for families.
  • Ensure service plans reflect assessments and client needs.
  • Provide crisis intervention and problem-solving assistance.
  • Monitor progress towards goals and adjust plans as needed.
  • Maintain accurate documentation of services.
  • Establish and maintain effective relationships with families and stakeholders.
  • Comply with HIPAA and other confidentiality regulations.

Required skills

  • Bachelor's degree in psychology, social work, counseling or related field.
  • Previous experience in mental health or social services.
  • Valid Texas Driver's license and auto liability insurance.
  • Knowledge of community resources and Medicaid policies.
  • Excellent organizational and multi-tasking skills.
  • Strong interpersonal and communication abilities.
  • Proficiency in Microsoft Office and Electronic Health Records (EHR).
  • Ability to work independently and as part of a team.
