Description: Since 1991, Cocoon House has been committed and working towards breaking the cycle of homelessness for teens, young people and their families throughout Snohomish County. The Youth Housing Case Manager, internally referred to as the Transitions Case Manager is responsible for creating and implementing case plans for the youth in Transitional Living Program for 16 and 17 year olds. As a key member of the Youth Housing team, the Case Manager works to ensure that youth in the Transitional Living Program are setting and maintaining progress toward goals in various domains including basic needs, life skills, education, employment, health, mental health, and substance use. You will be responsible for developing individualized plans to meet the needs of each youth, as well as providing guidance, advocacy, and professional support. The Case Manager should have a good understanding of the unique challenges facing youth in our community and be able to build strong relationships with them to ensure their success while maintaining healthy boundaries. Be able to assess potentially dangerous situations, deescalate and access emergency services if needed.
