Job Description

Overview

Cocoon House is seeking a Youth Housing Case Manager to support at-risk youth in their Transitional Living Program. This role involves creating individualized case plans, providing guidance and support, and ensuring the overall safety and well-being of youth aged 16 and 17 as they work toward their goals.

Key responsibilities

  • Maintain supervision of youth in care, ensuring safety and program compliance
  • Develop and manage individualized case plans for youth residents
  • Facilitate communication between clients, families, providers, and agencies
  • Make referrals for medical, dental, health, and substance use needs
  • Provide crisis intervention and de-escalation support
  • Assist clients in accessing necessary services and resources
  • Maintain accurate client records and documentation
  • Collaborate with staff and partners regarding case plans
  • Attend community meetings and transport youth to appointments
  • Participate in on-call rotation for support
