Overview

Cocoon House is seeking a Youth Housing Case Manager to support 16 and 17-year-olds in their Transitional Living Program. This role involves creating and implementing personalized case plans and providing guidance and advocacy to help youth achieve their goals in various life domains, such as education, health, and employment. The position requires strong engagement with the youth to foster success while maintaining professional boundaries.

Key responsibilities

  • Maintain high supervision of youth and ensure safety
  • Develop and manage individualized case plans for youth
  • Facilitate communication between clients, families, and agencies
  • Make appropriate referrals for medical, dental, and substance use needs
  • Provide crisis intervention and de-escalation
  • Assist clients in accessing necessary services and resources
  • Maintain accurate client records and documentation
  • Collaborate with staff and partners regarding youth needs
  • Attend community meetings and transport youth to appointments

Required skills
