Overview

The Physical Therapist - Home Health role at AMN Healthcare involves providing skilled physical therapy services to patients in their homes across Beckley, West Virginia. This position is focused on managing a caseload primarily of adult and geriatric patients, delivering personalized care, and ensuring they achieve improved functional mobility and quality of life.

Key responsibilities

  • Provide skilled physical therapy services to home health patients.
  • Manage a caseload of adult and geriatric patients with various diagnoses.
  • Conduct comprehensive evaluations and develop personalized treatment plans.
  • Implement interventions to improve patients' mobility, strength, and overall quality of life.
  • Educate patients and caregivers on exercise programs and fall prevention strategies.
  • Complete home safety evaluations and recommend modifications for independence.
  • Travel between patient homes for assessments and therapeutic interventions.
  • Collaborate with an interdisciplinary team to coordinate comprehensive patient care.

Required skills
