Overview

AMN Healthcare is seeking a Home Health Physical Therapist for a role based in York, Pennsylvania. This position involves providing one-on-one rehabilitative care to patients in their homes, focusing on regaining independence and safety following illness or injury. The role offers a combination of autonomy and collaboration with an interdisciplinary team to ensure holistic patient care.

Key responsibilities

  • Conduct initial and reassessment visits for patients
  • Develop and implement individualized treatment plans
  • Perform comprehensive patient assessments, including strength and balance testing
  • Educate patients and caregivers on mobility strategies and use of adaptive equipment
  • Document patient progress and care plans within the agency's EMR system
  • Collaborate with nursing and therapy team members for coordinated care
  • Recommend home modifications to improve patient safety
  • Manage a caseload primarily consisting of adult and geriatric patients

Required skills

  • Licensed Physical Therapist
  • Home Health experience (1 year preferred)
  • Familiarity with Homecare Homebase documentation
  • Knowledge of therapeutic exercises and manual therapy techniques
  • Experience in evaluating and developing treatment plans for diverse patient needs
