Overview

As a Home Health Physical Therapist at AMN Healthcare, you will provide personalized, evidence-based care to adult and geriatric patients in their homes in Albany, Kentucky. The role emphasizes autonomy and collaboration within a supportive interdisciplinary team, allowing you to improve patients' mobility and independence through comprehensive evaluations and tailored treatment plans.

Key responsibilities

  • Conduct in-home assessments and develop individualized treatment plans
  • Deliver interventions to improve functional mobility and support patient independence
  • Provide gait and balance training, educating patients and caregivers
  • Coordinate care with other healthcare professionals
  • Play a key role in fall prevention and chronic disease management
  • Document care activities electronically in a timely manner

Required skills

  • Physical Therapist license
  • Experience in home health care (1 year preferred)
  • Proficient in patient assessment, treatment planning, and exercise programming
  • Familiarity with Homecare Homebase documentation is a plus
