Overview

AMN Healthcare is seeking a Home Health Physical Therapist to provide personalized care to patients in their homes throughout Jeffersonville, Indiana, and surrounding areas. This role focuses on improving patients' functional independence, safety, and quality of life through evidence-based interventions and collaboration with an interdisciplinary team.

Key responsibilities

  • Deliver one-on-one, patient-centered care in patients' homes
  • Perform initial evaluations, revisits, and discharge assessments
  • Complete comprehensive assessments and establish individualized plans of care
  • Implement therapeutic interventions to improve mobility, strength, and balance
  • Conduct home safety evaluations and recommend adaptive equipment
  • Provide education on home exercise programs and fall prevention
  • Collaborate with an interdisciplinary home health team
  • Document care and communicate effectively in the EMR

Required skills

  • Physical Therapist license (Compact Accepted)
  • 1 year of home health experience
