Overview

The role of the Home Health Physical Therapist at AMN Healthcare involves providing one-on-one skilled therapy services to patients in their homes throughout the greater Pensacola area. This position emphasizes building relationships with patients to help optimize their function, independence, and safety while ensuring a holistic approach to care.

Key responsibilities

  • Conduct comprehensive initial assessments of patients
  • Develop and implement individualized plans of care
  • Provide gait and balance training, strengthening, and endurance programs
  • Employ pain management strategies and conduct home safety evaluations
  • Coordinate durable medical equipment needs
  • Collaborate with nurses, other therapists, and home health aides
  • Complete documentation using an EMR system
  • Travel between home visits within a defined territory
  • Focus on high-quality, individualized patient care and functional outcomes

Required skills

  • Licensed Physical Therapist in Florida
  • 1 year of home health experience
  • Familiarity with EMR documentation systems
  • Ability to work independently in the field
  • Experience with adult and geriatric patient populations
