Key responsibilities

  • Perform comprehensive physical therapy evaluations in patients' homes
  • Develop individualized plans of care and deliver evidence-based treatments
  • Educate patients and families on home exercise programs and safety strategies
  • Coordinate visits with patients and caregivers
  • Collaborate with an interdisciplinary team including nursing and occupational therapy
  • Manage a caseload focused on adult and geriatric patients
  • Travel to multiple patient homes within a defined territory

Required skills

  • Licensed Physical Therapist in Florida
  • 1 year of home health experience
  • Experience with homecare documentation
  • Knowledge of rehabilitation techniques and patient education
  • Familiarity with chronic disease management and post-acute care
