AMN Healthcare is seeking a Home Health Physical Therapist in Vero Beach, Florida. This role offers the opportunity to provide personalized care to patients in their homes, focusing on enhancing their mobility, strength, and independence through tailored rehabilitation plans. The position supports a diverse patient population in a scenic coastal community known for its relaxed atmosphere and rich cultural scene.

Key responsibilities

  • Perform comprehensive physical therapy evaluations in patients' homes
  • Develop individualized plans of care based on patient assessments
  • Deliver evidence-based treatments focused on strength, balance, mobility, endurance, and pain management
  • Educate patients and families on home exercise programs and fall prevention strategies
  • Coordinate visits with patients and caregivers and collaborate with an interdisciplinary team
  • Travel to multiple patient homes within a defined territory

Required skills

  • Licensed Physical Therapist in Florida
  • Minimum 1 year of experience in home health
  • Proficiency in patient assessment and care coordination
  • Experience with chronic disease management and post-acute care
  • Familiarity with Homecare Homebase documentation
