Overview

AMN Healthcare is seeking a Home Health Physical Therapist to provide skilled therapy services to patients in their homes throughout the Pensacola area. This role emphasizes building relationships with patients and families, allowing for personalized care and focus on improving function and independence in a supportive community environment.

Key responsibilities

  • Conduct comprehensive initial assessments and develop individualized plans of care
  • Provide gait and balance training, strength and endurance programs, and pain management strategies
  • Evaluate home safety and coordinate durable medical equipment needs
  • Collaborate with nurses, other therapists, and home health aides to align care plans
  • Document patient care using an EMR system
  • Travel between home visits to manage a caseload of primarily adult and geriatric patients
  • Participate in quality initiatives and pursue career advancement opportunities

Required skills

  • Licensed Physical Therapist in Florida
  • Minimum of 1 year home health experience
  • Familiarity with Homecare Homebase documentation
  • Strong patient assessment and documentation skills
  • Ability to manage caseload effectively
