Overview

AMN Healthcare is seeking a Home Health Physical Therapist based in Greenville, Alabama, who will join a dedicated interdisciplinary team focused on providing patient-centered care in a home setting. This role offers an opportunity to build meaningful relationships with patients and make a direct impact on their daily lives, all while enjoying the supportive environment and relaxed lifestyle of Greenville.

Key responsibilities

  • Perform comprehensive physical therapy evaluations in patients' homes
  • Develop and update individualized plans of care
  • Coordinate with physicians, nurses, and other health professionals
  • Provide skilled interventions including therapeutic exercise and home safety assessments
  • Educate patients and caregivers on equipment use and energy conservation
  • Conduct home safety assessments and recommend modifications
  • Manage a schedule of home visits and complete electronic documentation

Required skills

  • Physical Therapist license (PT)
  • 1 year of home health experience
  • Experience with adult and geriatric clients
  • Knowledge of orthopedic, neurologic, and cardiopulmonary conditions
  • Homecare Homebase documentation experience preferred
