Overview

The Home Health Physical Therapist role at AMN Healthcare is based in Eufaula, Alabama, offering therapists the opportunity to provide personalized care to patients in their homes. This position involves conducting assessments, developing treatment plans, and implementing interventions aimed at improving patient mobility, safety, strength, and independence.

Key responsibilities

  • Provide one-on-one physical therapy services in patients' homes
  • Conduct comprehensive patient assessments and develop individualized care plans
  • Implement treatment interventions focused on mobility, safety, strength, and independence
  • Complete in-home evaluations and design therapeutic exercise programs
  • Provide gait and balance training to patients
  • Recommend and educate on durable medical equipment and fall prevention
  • Cooperate with an interdisciplinary team for coordinated care
  • Document visit notes and track outcomes using an electronic medical record system
  • Travel between patients' homes within an assigned territory and adjust care plans as necessary

Required skills

  • Physical Therapist license
  • 1 year of experience in home health
  • Experience with homecare documentation systems such as Homecare Homebase
  • Patient assessment and treatment planning skills
  • Ability to create and progress therapeutic exercises
  • Gait training and home safety knowledge
  • Collaboration and communication skills
  • Documentation and electronic medical record skills
