Overview

This Occupational Therapist position is based in Eufaula, Alabama, focusing on providing home health therapy to patients. The role involves working with a diverse patient population to help maintain their independence and functionality through personalized care and rehabilitation strategies.

Key responsibilities

  • Provide Occupational Therapy services to patients in their homes
  • Perform comprehensive assessments and develop individualized treatment plans
  • Collaborate with an interdisciplinary home health team for coordinated patient care
  • Conduct visits focusing on functional mobility, ADL and IADL training, and home safety
  • Complete timely and accurate documentation using an electronic medical record
  • Participate in case conferences and care coordination meetings

Required skills

  • Occupational Therapist license
  • Home health experience
  • Knowledge of comprehensive assessments and treatment planning
  • Experience with diverse diagnoses including post-surgical recovery and neurological conditions
  • Proficiency in Homecare Homebase documentation (preferred)
  • Ability to maintain effective communication with a care team
