Job Description

Overview

The Home Care Nurse at Greater Baltimore Medical Center is responsible for delivering nursing and supportive care for terminally ill patients and their families in PG County. The role involves assessing patient and family care needs, implementing care plans, and providing direct nursing care while collaborating with interdisciplinary teams.

Key responsibilities

  • Completes initial assessment of patient and family to ascertain home care needs
  • Prepares and implements care plans in collaboration with the Interdisciplinary Group
  • Provides direct nursing care to maintain the highest level of care
  • Plans and supervises the delivery of care by Home Health Aides (HHA)
  • Updates primary physician and interdisciplinary team regularly
  • Teaches family members and facility staff patient care skills
  • Documents in facility chart and maintains communication with facility staff
  • Participates in care plan meetings for hospice patients
  • Provides educational in-services to facility staff
  • Participates in marketing and customer service activities

Required skills

  • Current Maryland State license as a Registered Nurse
  • Two years of nursing experience or equivalent education and experience
  • Knowledge of universal precautions for infection control
  • Strong time management skills
  • Excellent interpersonal and communication skills
  • Skill in the use of EMR (Electronic Medical Record)
  • Ability to be patient and family focused
