Job Description

Overview

The Support and Service Coordinator for the Children's Long Term Support Program (CLTS) at Wisconsin Community Services coordinates and facilitates access to support services for youth with developmental, emotional, or physical needs. This role involves working closely with families to develop individualized service plans that promote inclusion and self-empowerment, while ensuring the health and safety of the children in their communities.

Key responsibilities

  • Coordinate services with the child and family to meet developmental, emotional, and physical needs.
  • Provide family-centered, trauma-informed, culturally intelligent services through regular contact.
  • Assess and periodically reassess the child's health, safety, and functional capacity.
  • Identify high-risk children and provide enhanced care coordination.
  • Facilitate crisis intervention and resolution.
  • Develop and execute individualized service plans for families.
  • Authorize and manage support services through the CLTS Program.
  • Advocate for children and families to access resources and services.
  • Complete required documentation and adhere to policies and procedures.

Required skills

  • Bachelor's degree in a health or human services-related field
  • One year of experience working with individuals with disabilities
  • Strong knowledge of youth and young adults experiencing disability needs
  • Proficient with Microsoft Word and Excel
  • Ability to provide strength-based, family-centered services
  • Crisis response and intervention skills
  • Cultural competence and sensitivity
  • Valid driver's license and automobile insurance
