Job Description

Overview

The Speech Language Pathologist (SLP) at Life Care Center of South Hill provides direct rehabilitation care in speech therapy to patients following physician referrals. This role operates within Skilled Nursing Facilities, delivering both long-term and short-term rehabilitation services to a diverse patient population, while collaborating with an interdisciplinary team.

Key responsibilities

  • Provide direct speech therapy care based on physician referrals
  • Establish, assess, and modify therapy goals for patients
  • Collaborate with patients, families, and support departments to plan patient needs
  • Chart patient progress appropriately and timely
  • Utilize therapy software accurately
  • Assist in evacuation of patients when necessary
  • Maintain professional relationships and confidentiality
  • Participate in community health projects as needed

Required skills

  • Graduate of an accredited speech therapy program (MS or MA SLP)
  • ASHA certified or eligible for Clinical Fellowship Year (CFY)
  • Current state registration/licensure
  • CPR certification
  • Proficient in Microsoft Word, Excel, and email
  • Strong communication and collaboration skills
  • Knowledge of post-acute care laws and regulations
