Job Description

Overview

The SOR Pathfinder at the Helen Ross McNabb Center serves as a key member of a multi-disciplinary team focusing on engaging clients in substance use treatment and recovery. This role involves conducting assessments, maintaining connections with stakeholders, and ensuring proper follow-up and documentation for client care.

Key responsibilities

  • Engage clients in substance use disorder treatment and harm reduction
  • Conduct assessments and complete required documentation accurately
  • Track referrals and provide ongoing follow-up with clients
  • Maintain positive communication with internal and external stakeholders
  • Utilize motivational interviewing and best practice strategies to support clients
  • Stay current on community resources and referral processes
  • Assist with program evaluation and quality assurance indicators
  • Ensure compliance with HIPAA regulations and maintain confidentiality

Required skills

  • Bachelor's degree in Counseling, Addictions, Social Work, or a related field
  • Two years of experience working with substance use disorder populations
  • Knowledge of Motivational Interviewing and harm reduction strategies
  • Experience with cultural diversity and trauma-informed care
  • Computer proficiency for documentation and communication
  • Ability to operate a personal vehicle for job-related duties
