Job Description

Overview

The SOR Pathfinder at the Helen Ross McNabb Center is a critical role focused on helping clients engage with substance use treatment and recovery services. As a member of a multi-disciplinary team, the Pathfinder will conduct assessments, manage referrals, and provide ongoing support, ensuring that clients receive the necessary care and resources throughout their recovery journey.

Key responsibilities

  • Conduct client assessments and complete required documentation accurately and timely.
  • Track referrals and provide follow-up support to clients.
  • Utilize motivational interviewing and best practices to support clients 1:1.
  • Stay updated on community resources and referral processes.
  • Maintain two-way communication with team members and external stakeholders.
  • Complete government performance evaluations with the required completion rate.
  • Ensure adherence to HIPAA and confidentiality regulations.
  • Participate effectively as a member of a multi-disciplinary team.

Required skills

  • Bachelor's degree in Counseling, Addictions, Social Work, or related field
  • Two years of experience working with individuals with substance use disorders
  • Knowledge of Motivational Interviewing and harm reduction techniques
  • Computer expertise for documentation and communication
  • Understanding of cultural diversity and trauma-informed care
  • F-endorsement to operate a personal vehicle
