Job Description

Overview

The Service Coordinator for Lutheran Social Services in Wisconsin will work full-time to support residents across Tax Credit properties in Dane County. This role focuses on promoting housing stability and resident well-being by building relationships, completing assessments, and coordinating services.

Key responsibilities

  • Build strong relationships with residents, families, and community partners
  • Identify resident needs and complete assessments to develop Housing Stability Plans
  • Monitor service delivery and maintain communication with providers
  • Maintain on-site resource materials and educate residents on available services
  • Support residents through transitions with a planning process
  • Coordinate quarterly wellness/education sessions for resident support
  • Collaborate with on-site staff to identify resident needs and organize events
  • Track program outcomes, collect data, and prepare required reports
  • Perform additional duties as assigned

Required skills

  • Bachelor's degree in social work, sociology, psychology, or related field
  • Minimum of 3 years of related experience
  • Knowledge of supportive services in the Dane County area
  • Strong communication and relationship-building skills
  • Experience with case management and community outreach
