Overview

The Service Coordinator for Lutheran Social Services of Wisconsin & Upper Michigan will support residents in Tax Credit properties across Dane County, ensuring housing stability and well-being. This full-time position requires building relationships with residents, completing assessments, and coordinating on-site services.

Key responsibilities

  • Build strong relationships with residents, families, and community partners
  • Identify resident needs and develop Housing Stability Plans
  • Monitor service delivery and maintain communication with providers
  • Educate residents on available services and rights
  • Support residents through transitions with a proper plan
  • Coordinate quarterly wellness and education sessions
  • Collaborate with on-site staff to determine educational sessions and events
  • Track program outcomes and prepare required reports
  • Perform additional duties as assigned

Required skills

  • Bachelor's degree in social work, sociology, psychology, or related field
  • Minimum of 3 years of related experience
  • Experience with low-income, older adults, and people with disabilities
  • Knowledge of supportive services and resources in Dane County
  • Strong communication skills
  • Data tracking and assessment development
