Job Description

Overview

Lutheran Social Services of Wisconsin and Upper Michigan is seeking a full-time Service Coordinator for their Tax Credit properties located in Dane County. This role focuses on promoting housing stability and resident well-being by providing on-site services and support to residents.

Key responsibilities

  • Build strong relationships with residents, families, and community partners
  • Identify resident needs and complete assessments to develop Housing Stability Plans
  • Monitor service delivery and maintain communication with providers
  • Maintain on-site resource materials and educate residents on available services and rights
  • Support residents and families through transitions, ensuring plans are in place when exiting the property
  • Coordinate quarterly wellness and education sessions to enhance housing stability
  • Collaborate with on-site staff to welcome new residents and determine educational sessions and events
  • Track program outcomes, collect data, and prepare reports
  • Perform additional duties as assigned

Required skills

  • Bachelor's degree in social work, sociology, psychology, or related field
  • Minimum of 3 years of related experience, including work with low-income populations, older adults, and people with disa
  • Knowledge of supportive services and resources in the Dane County area
  • Strong communication and relationship-building skills
  • Experience in needs assessment, data tracking, and report writing
  • Ability to coordinate programs and community outreach
