Job Description

Overview

PathWays of the River Valley is seeking a Self-Directed Service Coordinator to support individuals with developmental disabilities in Sullivan and Lower Grafton counties, NH. This role involves working collaboratively with individuals and their support teams to design and monitor personalized support services while promoting individual choice and decision-making authority.

Key responsibilities

  • Carry a caseload of 25-30 individuals.
  • Establish and maintain a fluid schedule of quarterly visits and monthly contacts.
  • Facilitate person-centered annual planning meetings.
  • Complete and track all regulatory compliance requirements.
  • Develop individual budgets and monitor implementation through financial statement reviews.
  • Address quality concerns in cooperation with team members.
  • Understand the needs and circumstances of individuals and families.
  • Maintain knowledge of Medicaid waivers for developmental disabilities.

Required skills

  • Bachelor's degree in human service or related field preferred.
  • 2-3 years of case management or similar experience.
  • Experience with developmental disabilities preferred.
  • Valid driver's license and insurance with reliable vehicle access.
  • Proficiency with documentation and MS Office (Word, Outlook, Excel).
  • Ability and willingness to learn new programs.
