Provider Healthcare is seeking a skilled and passionate RN Team Nurse to join a cohesive interdisciplinary team in New Holland, Pennsylvania. This full-time position involves providing direct patient care through both in-person and telemedicine, while collaborating with various healthcare professionals to ensure excellent patient-centered care.

Key responsibilities

  • Provide direct patient care as an RN through in-person and telemedicine
  • Conduct pre-visits, post-visits, and patient education
  • Develop care plans and manage chronic care
  • Carry out orders for prescriptions and labs
  • Act as a communication line between hospitals, primary care providers, and specialists
  • Complete documentation efficiently according to policy
  • Collaborate with clinicians to support at-risk patients

Required skills

  • Current Pennsylvania Registered Nurse license
  • 1-2 years of clinical experience as an RN
