Overview

Provider Healthcare is seeking a collaborative and goal-oriented RN Team Nurse to join an interdisciplinary team in Lebanon, Pennsylvania. This full-time position involves providing direct patient care both in-person and via telemedicine, while fostering communication among healthcare providers and patients.

Key responsibilities

  • Provide direct patient care through in-person visits and telemedicine
  • Conduct pre-visits, post-visits, and patient education
  • Develop care plans and manage chronic care
  • Collaborate with clinicians to support at-risk patients
  • Efficiently complete documentation according to policy
  • Act as a communication line between healthcare providers and patients

Required skills

  • Current Pennsylvania Registered Nurse license
  • 1-2 years of clinical experience as an RN
  • Excellent oral and written communication skills
  • Ability to work one evening if required
  • Skills in time management and team leadership
  • Experience in telemedicine and patient care coordination
