Overview

Enhabit Home Health & Hospice is seeking a Full-time weekend RN to join their team in Reston, Virginia. The role focuses on providing skilled nursing visits and coordinating care for patients at home, emphasizing innovative and collaborative patient care in a stable workplace culture.

Key responsibilities

  • Perform skilled nursing visits and coordinate client care
  • Act as the point of contact for all disciplines involved in patient care
  • Oversee the frequency of visits for the patient care episode
  • Consult with the physician regarding patient care details
  • Ensure accurate OASIS collection and medication profile updates
  • Report lab values to the physician in a timely manner
  • Attend weekly case conferences and monthly case manager meetings

Required skills

  • Current RN licensure in the state of employment
  • Minimum of one year of clinical experience preferred
  • Knowledge and skills in current nursing practice
  • Valid state driver's license and automobile liability insurance
